North West Surrey CCG introduced its innovated locality hub model in 2015 with the launch of the Bedser Hub in Woking Community Hospital. The Hub, which is now run by NHS community provider CSH Surrey, provides integrated health, social care, mental health and voluntary sector services to proactively keep patients independent.

The benefits of the Hub as highlighted in the Future Hospitals Reports include reduced admissions to acute hospitals, improved patient experience and feedback, improved processes and financial efficiencies.

The Future Hospital Programme (FHP) was established by the Royal College of Physicians in response to the seminal report Future Hospital, Caring for Medical Patients from the Future Hospital Commission, published in 2013.

The Future Hospital Programme included eight Future Hospital development sites (selected, supported local healthcare project teams); a pilot of a new role of chief registrar (a senior clinical leadership role for experienced trainee doctors); and other workstreams relating to person-centred care, young adults and adolescents and integrated care.

Sarah Tomkins, Associate Director of Operations at CSH Surrey says: “The Bedser Hub sees more than 900 people per month who are referred by their GPs or other professionals and has a total caseload of 2,200.

The Multi-agency Team of GPs, Speciality Consultants, nurses, therapists, social workers, mental health professionals and voluntary sector colleagues assess people and then work together to put in place relevant health and/or care plans and other support that means that people can continue to live independently.

The success of this service is the collaboration of a fantastic team of people from across many disciplines who together provide this innovative, integrated service. In 2018 we will be

extending this successful model to two other localities in the north west Surrey area, meaning more people will benefit from integrated care and support.”
